I've played in a game that uses most of the combat options on an ad-hoc basis - i.e. if a players says 'I want to accomplish this' and it's covered by one of the combat options but not the basic rules, then the DM usually uses those rules or some other extrapolation.
The same campaign also uses Lingering Injuries in an interesting way. Once per session (i.e. not once per player, but once in total), while dying a PC may opt to take a random Lingering Injury to automatically stabilise instead of rolling a death save. Personally I'm not a fan of this specific house-rule (I'd rather see them used for massive damage), but the other players mostly enjoy a bit of the old subtle PvP action so it generally goes down well with them.
Put me in the group that thinks Sanity checks for any spellcasting is a bit too much.. but I think they might work well for ritual casting. I.e. make a check any time you cast a spell as a ritual.

- Just wanted to get people's take on the Lingering Injuries optional rule in the DMG (p. Background to the question: initially, I was well-disposed toward this rule, because I like grittier, AD&D-style combat (and injury recovery rules).
- This document serves as a modification to the lingering injury rules as found in the D&D5e Dungeon Master's Guide. These rules provide a 'tier' effect to injuries suffered, reducing the risk of suffering debilitating injuries on a single roll. Especially useful if you wish to add a bit of risk and danger when players are knocked unconscious.
- I currently don't use lingering injuries, instead I abstract dropping to zero with causing a level of Exhaustion. I am thinking about trying it out though. Don't use Massive Damage - I feel it can penalize low level characters too greatly. I don't use Morale as presented here, I steal morale and reaction rules, whole cloth from B/X.

Ark dmg cap. Optional rules for Lingering Injuries can be found on page 272 of the DMG and be used at the DM's discretion. Healing can remove the injury as written, but a DM could always rule otherwise. Yes any rules variant in the DMG are not in the vanilla version of the game.
